Don't recreate the wheel! Forms that were already created in Forms Manager can be imported into the new Forms module. And they can be imported more than once, so don't be afraid to bring them over! You can always import again and start with a clean slate.
To import a form, select the dropdown to the right of the green "Create Form" button, then select "Import from Forms Manager."
This brings up a dialog box allowing the form to be selected from the dropdown list. (Note that the form will be imported into the "Uncategorized" folder and can be moved at a later time.) Select the form and click "Import."
Forms has a limit of 100 fields per form. If an imported form has more than 100 fields, a warning will display and the form cannot be saved until you delete excess fields.
Form submissions
Only the form structure is imported; all submission data will stay with the original form. This means you can also continue to edit and use the form in Forms Manager after it's been imported. The imported form works independently from the form in Forms Manager and is not connected in any way.
Form field types

Name and address fields are converted to a text (Short Answer) field.

Single Choice, Multiple Choice, and Dropdown fields import with all choices included. If an "Alternate Results Label" is used, it is ignored and will need to be redefined in the field settings. If a "default choice" is selected, it is respected in the field settings. If a "Please select" option was included, it will not be defined in the settings.

Conditional fields themselves are imported, but conditional rules will need to be reapplied.

Registration fields convert to corresponding field types (e.g. Single Choice). The functionality of registration elements is not yet available in Forms.

Fields used for currency may need to be switched out for a true "Currency" field type to define pricing. Remember, if a field is supposed to be used for pricing, it should have the pink "Price" indicator. When a field is defined with Pricing, the Payment Total box automatically displays, which means you don't need to add a field to "display totals from currency fields." If this field is included in the imported form, it is converted to a text field. Make sure payment processing has been defined for Forms.

Date fields get imported as is, but be sure to click on the field and check out the new field options, including accepted dates and days, to be selected.

Hidden fields, Attach File fields, and Page Breaks do NOT import, as these are not currently options in Forms.
Field properties

If a field uses Prefill options, they will need to be redefined in the field settings under "Advanced Settings."

If a field uses Validation or Size Options, they will need to be redefined in the field settings.

If a field uses default values, the value is not imported and should be noted in the "Placeholder Text" option in the field settings.

Description text gets converted to "Hint Text." If there is no description text, the the Tooltip text is converted to Hint Text. If both Tooltip and Description are populated, only the Description text is used. Note: Hint Text has a character limit of 100 characters. If the text exceeds this amount, an alert will appear upon saving the form. A workaround is to insert a Content field to house the text.

The field label and reporting label get imported without changes.
Submit button
The Submit button text will be imported without changes and can be updated by clicking on the Submit button section. A major improvement in Forms is that payment processing now happens on the same screen as the form submission, so it's no longer necessary to change the submit button to say "Proceed to Payment."
Notifications and redirects
Form notifications, if applicable, also need to be reviewed in the imported form.

Admin Notifications: If Admins were to be notified, these are still included in the imported form. If "Include Full Results" was enabled in Forms Manager, "Include Submission Details" is toggled on.

Submission Options: If "Redirect to a site page" is defined, this is included. If "Display a message" is defined, the message is included, but it is now limited to text only and up to 255 characters. "Redirect to URL" is NOT imported.

Respondent Notifications: If the "Send Confirmation email" setting was enabled, then "Send to Submitter" is enabled in the new form. If "Include full results" was enabled, then "Include Submission Details" is enabled.
Confirmation Email Fields are no longer a separate field displayed at the bottom of the form. Instead, an Email Address field will need to be inserted into the form and defined in the Submit/Notification settings. If more than one Email Address field is included in the form, the email to be used can be selected from the dropdown list in the notification settings. You may still want to add an email field at the bottom of the form labeled "Please send a confirmation email to the following address" if you've collected multiple addresses and want to make sure the best one is identified.
